Population Health Management

Evaluaide is a population health management program designed to evaluate the overall health of a plan population and identify participants who have, or are at risk for developing, common and costly chronic health conditions.

Evaluaide nurses work with participants to develop individualized healthcare goals and plans of care. Customized care plans with participant centered goals will optimize participant involvement in managing their chronic health conditions. The program is designed to increase patient compliance with prescribed care regimens including: blood glucose testing protocols, medication compliance, and diet & exercise recommendations. Additionally it can reinforce chronic health condition-specific education through written materials and one-on-one conversations with RNs.

The Evaluaide nurses will work collaboratively with providers to help manage high risk patients. It is like adding a nurse to your staff and may take some of the burden from office staff by providing patient follow-up and support both telephonically and with home visits, if required.
